For years, MSG has carried a rather stubborn reputation. Headaches, uneasiness, the old phrase “Chinese Restaurant Syndrome” – these claims have followed it around for decades.

The science, however, has been far less certain.

To understand the link more clearly, researchers reviewed human studies on MSG and headaches. The study, “Does Monosodium Glutamate Really Cause Headache? A Systematic Review of Human Studies,” looked at whether MSG intake could be convincingly tied to headache occurrence.

The answer was not a simple yes.

Examining the Scientific Evidence

The researchers reviewed clinical studies where people consumed MSG orally. They made one important distinction: MSG taken with food, and MSG taken without food.

That distinction is pivotal.

The review included ten eligible studies. Five looked at MSG consumed with food. Another five papers, covering seven studies, looked at MSG consumed without food. The researchers then compared headache reports between MSG and placebo groups.

What the Studies Found

The food-based studies were fairly consistent. In five human studies, MSG was consumed as part of food, in doses ranging from 1.5 to 3.15 grams.

Across these studies, researchers did not find a significant increase in headaches compared with placebo, except in one subgroup from a single study. Taken together, the evidence did not show a clear link between MSG eaten with food and headaches.

The studies without food told a messier story.

In these studies, MSG was given in beverages or soup, with doses ranging from 1.25 to 12 grams. The concentrations were also high – between 1.125% and 12%.

That is quite far from everyday use.

At concentrations above around 1.3%, MSG develops a strong, distinctive taste. This made blinding difficult. Participants may have been able to tell whether they were getting MSG or placebo simply from taste.

Once that happens, the findings become harder to read. Expectations can creep in. So can discomfort from consuming a strong MSG solution without food. Either could have influenced headache reports.

The authors noted this concern clearly. Large amounts of MSG dissolved in liquid, without accompanying food, do not reflect the way MSG is usually consumed in meals.

Why Study Design Matters

This is the central point of the review: design matters.

Many of the studies that reported headaches used MSG in concentrated beverage solutions, often without food. These conditions are not typical of normal eating. They may also have made it difficult to keep participants properly blinded.

If someone can taste the difference between MSG and placebo, the study becomes less reliable. It does not mean the findings are useless. It means they need to be treated carefully.

There is another practical issue. Highly concentrated MSG solutions may cause gastrointestinal discomfort in some people. That discomfort alone could play a role in reported headaches.

By comparison, studies where MSG was consumed with food and in more realistic dietary amounts generally did not show a significant rise in headaches. This means that context is key when deriving conclusions from studies.

Thus, the current scientific evidence does not establish a clear causal relationship between MSG and headaches.

When MSG was consumed with food, in doses from 1.5 to 3.15 grams, the evidence did not consistently show more headaches than placebo. Many positive findings came from studies using concentrated MSG solutions, ranging from 1.125% to 12%, with doses as high as 12 grams, given without food.

That distinction is important. It is the difference between MSG as part of a meal and MSG tested in conditions most people would never encounter.

The authors of “Does Monosodium Glutamate Really Cause Headache? A Systematic Review of Human Studies” called for better-designed studies before any firm causal link can be made.

For now, the review suggests that some long-held beliefs about MSG and headaches lack any tangible evidence behind them, especially when MSG is studied in ways that resemble normal eating.
